Five Stars

Five-Star Quality

All of Mosaic’s main clinics eligible for assessment have received a five-star ranking from the Patient Centered Primary Care Home Program. A five-star status is the highest recognition of clinical excellence and innovation a primary care clinic can receive. In Oregon, less than 5% of all primary clinics have achieved this designation.

In 2022 the Health Resources and Services Administration (HRSA) Bureau of Primary Health Care presented Mosaic with a “Community Health Quality Recognition” honor in recognition of ranking among the top 30% of health centers in the nation for best overall clinical performance.

Our History

Responding to community need
First Mosaic Clinic in Prineville

In the spring of 2001, a family in Prineville without insurance lost their infant to a preventable disease. The tragic loss happened amid a surge in emergency room usage for basic illness that had gone untreated.

Stirred to action, local leaders banded together and in 2002 established Central Oregon’s first Community Health Center—the Ochoco Community Clinic—which would later evolve into Mosaic. They created a private, nonprofit organization and chose patient and community leaders to guide the community health center as the first Board of Directors. 

During our first year, we provided affordable preventative healthcare to nearly 700 individuals. Word spread quickly about the value of the Community Health Center model and by 2006 we were operating clinics in Crook, Deschutes and Jefferson Counties.

Mosaic Today

Expanding to meet demand

Today, through a network of more than a dozen health centers, Mosaic serves a patient base of 27,000+ individuals in Bend, Redmond, Madras and Prineville.

Our organization employs nearly 400 staff across the region, representing the demographics of those we serve. We are governed by a volunteer, patient-majority Board of Directors. And our CEO, Megan Haase, FNP, is one of the original providers who was part of the launch of the Prineville clinic at the heart of the Mosaic legacy.

Ever since we opened our doors in 2002, we have continually invested in meeting patient and community needs with new facilities, expanded services, and top-notch, dedicated providers. Now as we enter our third decade of service, we remain committed to advocating for our patients and continuing to innovate in providing the high-quality healthcare everyone deserves.
