Welcome to Mosaic! In preparation for your visit, please review the following information.

Medical Chaperones

Beginning July 1, 2023 the Oregon Medical Board requires that providers offer patients the opportunity to have a trained medical chaperone available for sensitive exams.

Medical chaperones are trained to know what to expect during an exam and are there to ensure patient and provider safety. A chaperone is offered for your comfort, but you may choose to decline to have a chaperone present. Please speak with your healthcare provider if you have questions about a sensitive exam or the use of a medical chaperone.

Preparing for your visit: New Patients

 

What to bring with you

  • Insurance Card (if you have insurance)
  • Proof of Income (required for application to our Sliding Scale discount program)
  • Photo Identification (or an item with your current address such as a bill or bank statement)
  • All medications that you are currently taking (bring the bottles/containers)

You will be asked to complete documents at your visit. Information about patient forms may be found here.

If you are unable to complete these documents because of difficulty reading or writing, a community health worker can assist with this process. Please contact us at 541-383-3005.

Preparing for your visit: Existing Patients

Please bring the following items with you to your Mosaic visit:

  • Insurance Card (if you have insurance)
  • Photo Identification (or an item with your current address such as a bill or bank statement)
  • All medications that you began taking since your last visit (bring the bottles/containers)

Preparing for your visit: Immigration Medical Exams

Before non-U.S. citizens can be approved for an adjustment of status, the U.S. Citizenship and Immigration Services (USCIS) requires a medical exam, commonly called an INS physical but officially named the USCIS Immigration Medical Exam. This exam must be completed by a government-authorized physician known as a Civil Surgeon.

Mosaic provides INS physicals for any non-U.S. citizen, regardless of whether you are an established patient or which state you live in.

Anyone can be a Mosaic patient—insured or uninsured, working or unemployed.

Sliding Scale Discount Program

Whether you have insurance or not, you may be eligible for our Sliding Scale discount program and qualify for free or reduced healthcare.
