PGY2 Ambulatory Care Pharmacy Residency Program

Mosaic’s PGY2 Ambulatory Care Pharmacy Residency Program builds on Doctor of Pharmacy (Pharm.D.) education and PGY1 pharmacy residency program to contribute to the development of clinical pharmacists in Ambulatory Care. PGY2 residencies provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care and other advanced practice settings. Residents who successfully complete an accredited Ambulatory Care PGY2 pharmacy residency are prepared for advanced patient care, academic, or other specialized positions, along with board certification, if available.

By the end of the residency program, the resident should be able to operate as an independent practitioner in the setting of ambulatory care. The resident should be skilled in developing and utilizing collaborative practice agreements, be comfortable prescribing medications for chronic disease states, and providing comprehensive medication therapy management to patients. The resident should be adept at researching evidence-based medicine questions, providing health education and have professional development planning abilities.

The clinical pharmacy resident will complete required rotations in Primary Care, Medication Therapy Management, Health Education, Research, Practice Management and Professional Development, and 340b Community Pharmacy. Primary Care rotations are all encompassing and include collaborative drug therapy management of diabetes, hypertension, ASCVD risk reduction, smoking cessation, anticoagulation, and Hepatitis C. Primary Care rotations also support our team based care model and provide services like polypharmacy reviews, medication optimization services, and drug-interaction inquiries. Primary Care also offers experience in an ambulatory internal medicine clinic, controlled substance management, evidence based medicine, and drug information services. The resident will have elective opportunities including a deeper dive into Pharmacy Administration, Informatics (e.g. becoming an Epic SuperUser), Mobile Van Care, and Behavioral Health.
