Who can use the Mosaic Pharmacy?

Anyone can use the Mosaic Pharmacy. You do not have to be a Mosaic patient – we are open to everyone.

Does the Mosaic Pharmacy offer sliding scale pricing?

Sliding scale pricing is available to Mosaic patients. Discounts are based on income limits set by the federal government

Are there other ways to get discounts at the Mosaic Pharmacy?

Yes. Mosaic offers a Prescription Club that provides members with access to thousands of medications at a discounted rate (including 300 generic medications capped at $4). Anyone can join the club for an annual enrollment fee of $10 per family.

What if I have insurance but still need financial assistance?
Our Sliding Scale Discount Program is open to those with and without insurance. Even if you don’t think you would qualify for the program we encourage you to apply. Submitting the application may open up other opportunities within Mosaic to financially support your care.
How can I transfer my prescriptions to the Mosaic Pharmacy?

A member of the pharmacy team can help you transfer your prescriptions. Visit or call us with your photo ID, insurance card and list of medications ready. Our team will take care of the rest.

What insurance plans does the Mosaic Pharmacy accept?

The pharmacy accepts most major insurance plans (OHP, Medicare and Private/Commercial).

Can I manage my prescriptions online?

Yes. Customers can request refills and receive notifications online or on mobile devices through an app called MyChart. Visit the MyChart FAQ page to learn how to enroll.

How are Mosaic Pharmacies preventing the spread of COVID-19?

Customers exhibiting COVID-19 symptoms (fever, cough, and/or shortness of breath) can have medications delivered to their vehicles by calling the pharmacy ahead of time. Pickup may be expedited by prepaying any copays over the phone using a credit or debit card.

Does Mosaic Offer mail delivery of medications?

Mosaic Pharmacy offers mail-delivery of some prescription medications. Talk to a pharmacy team member to learn more.

Clinical Pharmacy Services

Mosaic’s in-house clinical pharmacists meet with patients during scheduled appointments to answer medication questions and help manage their chronic conditions alongside their Primary Care Provider and Care Team. Common conditions our Clinical Pharmacists see patients for include Anticoagulation, Diabetes, Hypertension. The clinical pharmacy team is integrated into these primary care clinics as part of a team-based care model. They offer both in-person and telephonic visits.

PGY2 Ambulatory Care Pharmacy Residency Program

Mosaic’s PGY2 Ambulatory Care Pharmacy Residency Program builds on Doctor of Pharmacy (Pharm.D.) education and PGY1 pharmacy residency program to contribute to the development of clinical pharmacists in Ambulatory Care. PGY2 residencies provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care and other advanced practice settings. Residents who successfully complete an accredited Ambulatory Care PGY2 pharmacy residency are prepared for advanced patient care, academic, or other specialized positions, along with board certification, if available.

By the end of the residency program, the resident should be able to operate as an independent practitioner in the setting of ambulatory care. The resident should be skilled in developing and utilizing collaborative practice agreements, be comfortable prescribing medications for chronic disease states, and providing comprehensive medication therapy management to patients. The resident should be adept at researching evidence-based medicine questions, providing health education and have professional development planning abilities.

The clinical pharmacy resident will complete required rotations in Primary Care, Medication Therapy Management, Health Education, Research, Practice Management and Professional Development, and 340b Community Pharmacy. Primary Care rotations are all encompassing and include collaborative drug therapy management of diabetes, hypertension, ASCVD risk reduction, smoking cessation, anticoagulation, and Hepatitis C. Primary Care rotations also support our team based care model and provide services like polypharmacy reviews, medication optimization services, and drug-interaction inquiries. Primary Care also offers experience in an ambulatory internal medicine clinic, controlled substance management, evidence based medicine, and drug information services. The resident will have elective opportunities including a deeper dive into Pharmacy Administration, Informatics (e.g. becoming an Epic SuperUser), Mobile Van Care, and Behavioral Health.

Staffing/Duty Requirements

One half day shift every other week at one of our community retail pharmacies. Currently this shift takes place during the normal work week.

Call Requirements

There is no on-call requirement associated with this residency.
