Our school-based health centers function as independent pediatric healthcare clinics located on school grounds
School-based health centers provide convenient access to care for children aged birth through 18. Your child does not need to attend the school to receive services. Simply find the location nearest you and call 541-383-3005 to make an appointment.

Mosaic Community Health, Sisters School District and Deschutes County Health Services are expanding access to critical substance use disorder (SUD) treatment for adolescents across Central Oregon, thanks to new funding from the state’s Rural Health Transformation Program.
Mosaic’s newly located Bend High School-Based Health Center reopens in March 2026, offering accessible pediatric medical and behavioral care for youth 0–18.
