COVID-19 Test Scheduling

Mosaic is able to do limited, rapid COVID-19 testing for Mosaic patients only at this time.

We will ONLY be testing those who:

• Have symptoms of COVID-19 including:
  • • Fever or chills
    • Cough
    • Shortness of breath or difficulty breathing
    • Fatigue
    • Muscle or body aches
    • Headache
    • New loss of taste or smell
    • Sore throat
    • Congestion or runny nose
    • Nausea or vomiting
    • Diarrhea
• Do not have symptoms of COVID-19 but who have been in close contact (within six feet for a total of 15 minutes or more) with someone with confirmed COVID-19 and it has been at least five days since the initial exposure but not more than 14 days since the last exposure.

Testing is also available for Mosaic patients only who require COVID-19 testing before a medical procedure or surgery.

NO walk-in testing
NO testing for travel
NO testing for “return to work”
NO testing for concert or other venue requirements
